How is everyone handling In-App purchases? We are trying to figure out a way to centrally manage these purchases but have found no solution thats viable.

There is no viable solution.
What I do is writing messages to all the developers of such applications and beg them to release a second version of their app where there is no in-app purchase but costs as much as all the in-app purchases and includes all the features. The feedback I get is often very positive. This app programmers often don't know that there are MDM solutions and that all this clients can't distribute (=buy) their apps.
