Is there a way to suppress this message? On iPads using a shared account, I have to click "OK" dozens of times.

If you can hold out until this fall, Apple is doing away with having to use Apple IDs on shared iPads. You will be able to push apps directly to iPads without it having an AppleID signed in. WWDC Video explaining it. For now, I guess if you used a Configuration Profile that would install immediately after enrollment that disables FaceTime (and other supervised features) should stop that from showing up, but you'd have to test that.
