Looks like its forgotten the password. You can edit the DB connection settings in the utility. Try re-inputting the MySQL password.

Thanks for the reply. If i click OK the next pop up is the attached. I knew what the Password is but is this the correct user?
Is there any way I can double check the database name and username that I should be inputting?

The details will be stored in the DataBase.xml file on your server. It should be in the program files / JSS folder if its running on Windows.
If the file doesn't have the information, you should restore a previous copy to check the database name, username and password values.
The ones in the screenshot are the defaults so may be correct depending on how the JSS was installed.
There is of course another possibility. Something like a firewall or port setting changing during an upgrade process. In case you get to a point where you're sure the DataBase.xml settings are correct.
