That is not uncommon. You could try asking your Apple sales rep to see if they can follow up, but it's not likely that you'll see a quick result.
For anecdotal reference, we submitted the SSA application sometime around November 2014, and it was at least July/August of the next year (with intermittent communication throughout) before we were sent a contract to sign. It took another few months of negotiation between our legal department and Apple's contract team to get things worked out and to get our account set up. All in, the process took about a year.
