I may just be overlooking it, but where do I associate a Per App VPN connection with an app to require that app to use the VPN connection? I found where to setup the VPN, but not the app to associate it with.

Check in Mobile Devices> Apps. There, at the bottom you can setup the app to use per app vpn.
