Non DEP IPad 2 will not appear properly in JSS

It looks like during a test or old trial for JSS mobile devices, this device appeared.
I have reset it.
deleted every instance, I could find based on the serial number.
Prepared it using Apple Configurator 2.
Apple configurator shows it as supervised but the JSS inventory still show it as unmanaged and unsupervised.
It will not install the apps even If I manually add it to the correct department and building for the scope.
It consistently reappears under the old name and as unmanaged and unsupervised.
